- Switch on Required for billing to hold a visit's invoice until this form is approved.
- A choice question — Multiple choice (pick one), Dropdown (pick one from a list) or Tick boxes (tick any): write each option on its own line. Add an option, or press Enter, for the next; pasting several lines adds several; move or remove each with the arrows and the bin.
- Text under the heading: under each section's title, write instructions or a note for whoever fills it in. It shows under the heading on the form, on a phone, and on the printed copy.
